What you will do:

Provide specialist social work support and intervention to individuals and families experiencing homelessness.
Conduct comprehensive assessments to identify needs, risks, strengths, and appropriate support pathways.
Develop and implement effective case management plans tailored to individual circumstances.
Work collaboratively with local authorities, charities, housing providers, and other agencies to secure sustainable housing solutions.
Advocate for service users, ensuring they can access appropriate housing, health, social care, and community resources.
Manage complex cases and provide professional guidance to colleagues where required.
Maintain accurate case records and documentation in line with statutory and organisational requirements.
Identify and respond appropriately to safeguarding concerns.
Promote independence, resilience, and positive outcomes for individuals and families.
Contribute to multi-agency meetings and partnership working to improve service delivery.
