The Living Well Service provides holistic, physical, emotional, psychological, and spiritual care and support to people living with advanced or terminal illness; who are stable or slowly deteriorating. This care is provided in a place that is relevant to patients needs and could be on either of our sites, with peers in a group, as an outpatient, virtually or with a community or home visit. The successful applicant will work as part of the North East Hampshire, Guildford, Waverley, and Surrey Heath multi-disciplinary locality teams and in partnership with community services, specialists, and primary care to ensure care is coordinated, holistic and person-centred. The postholder will support patients to manage symptoms, take a proactive approach to enabling choices and supporting self-management, and be supported to develop their skills in palliative care. The successful applicant must be flexible and able to work between our sites at The Beacon and at The Hospice in Farnham or in other service areas according to patient need. The postholder will work as a member of the Phyllis Tuckwells (PT) Community Palliative Care Team. They will maintain the highest standards of nursing care with patients, their families, and carers. This role will involve holistic assessment, identification of patient and carers needs, care planning and care provision jointly with patients, families, carers and other multidisciplinary team members. The service is complementary to, and dovetails with, other health and social care services within Guildford, Waverley, North East Hampshire, Farnham and Surrey Heath so that people who use our service receive co-ordinated and person-centred care. The role requires a commitment to enabling choice, supporting self-management, enabling rehabilitative palliative care and facilitating respectful and dignified support for patients, their families and carers. The postholder will support a persons symptom control needs. The role will contribute to a safe, effective, and efficient culture by contributing to risk assessment and by working in accordance with PT policies and procedures. The Living Well Service provides holistic physical, emotional, psychological, and spiritual care and support to people living with advanced or terminal illness who are stable or slowly deteriorating. - Current NMC registration in adult or general nursing.

- Have well developed communication, interpersonal and team working skills.
- Be willing to undertake further training in communication and palliative care skills.
- Demonstrate understanding of community working.
- Be self-aware, resilient, and able to cope with stressful situations.
- Have post registration experience.
- Have experience of working with people approaching the end of life.

1.0 Clinical
- 1.1 To assess, plan, implement and evaluate a care plan, according to a patients choices and their changing needs in conjunction with colleagues internal and external to Phyllis Tuckwell (PT).
- 1.2 To work as part of the multi-disciplinary team, providing relevant holistic support to the patients, their families, and carers.
- 1.3 To provide care in a relevant setting including the patients home, as an outpatient, virtually, by telephone or in a health, social care or community venue.
- 1.4 To provide the opportunity for patients to access support according to need and choice. This may include: One to one attendance, with carers, with other health professionals or with a group of peers; Signposting and enabling access to other services including primary care, social care and other providers; Identifying patient and family needs as they change and involving colleagues as required.
- 1.5 To ensure that patients and their families have access to resources that will enable them to manage their concerns and needs. To enable access to advance care planning.
- 1.6 To ensure patient records are properly maintained in accordance with NMC guidelines and PT policy.
- 1.7 To be able to demonstrate an understanding of the ethical aspects of palliative care and their application in the workplace.
- 1.8 To report accidents, incidents, and complaints to the Team Lead/ Manager in a timely way.
- 1.9 To maintain competencies in medication management and understand and adhere to the PT and Community Medicines Management policy in relation to the correct handling, usage and administration of all drugs.
- 1.10 To act as a resource, providing clinical guidance and support to colleagues.
- 1.11 To ensure statutory obligations are met and followed as relevant to the clinical area.
- 1.12 To ensure all PT policies are followed, quality standards are maintained and constantly re-evaluated, to provide the highest standards of holistic care.
- 2.1 To understand and work within the relevant legislation and guidance, and according to the PT vision, values, mission, and philosophy.
- 2.2 To work as part of the multidisciplinary team, providing holistic care to patients, their families, and carers.
- 2.3 To provide appropriate information and support to patients carers, friends and other professionals
- 2.4 To liaise and develop effective relationships with external health care professionals and other agencies, ensuring patient continuity across all care settings.
- 2.5 To be accountable for own professional practice within the NMC Code of Conduct.
- 3.1 To ensure a safe working environment is maintained, in accordance with the PT Care Fire and Health and Safety policies, reporting any potential hazards to the line manager.
- 3.2 To report any feedback, concerns and complaints same day from patients, their relatives, or other advocates to the Team lead/Line Manager as appropriate
- 3.3 To ensure that the nursing resources are used wisely.
- 3.4 To ensure team cohesion and spirit is maintained, through ongoing supportive relationships and consideration of colleagues always.
- 3.5 To adhere to the lone working policy in the community.

- 4.1 To share own knowledge, skills and learning with colleagues and embed in patient care.
- 4.2 To develop an area of clinical expertise and disseminate to fellow team members as a specialist resource.
- 4.3 To support the professional development and safe practice of others.
- 4.4 To maintain up-to-date clinical experience and knowledge and be able to provide evidence of this for revalidation folder.
- 4.5 To act as an assessor and mentor to students who need to develop skills and acquire experience in the field of palliative care.
- 4.6 Through PT annual appraisal, encourage professional growth and identify areas for development.
- 4.7 To strengthen and develop clinical skills across all care settings with the support of a line manager.
- 5.0 To participate in quality improvement initiatives, research and audit within the PT environment, and encourage collaboration.
- 5.1 To implement evidence-based practice.
Role holders are required to understand their responsibilities in the safeguarding context, ensuring that Phyllis Tuckwell reduces the risk of harm or abuse of adults and children at risk. You are required to:
- adhere to Phyllis Tuckwells Safeguarding Policy (held on the NEST), which supports the local authoritys multi agency safeguarding policy, the law (Care Act 2014) and Mental Capacity Act 2005 (including Deprivation of Liberty Safeguards).
- inform the appropriate PT professional where there is concern that an adult or child may be at risk of harm, abuse, or neglect.
