Job summary
We are looking for a highly motivated Practice Nurse to join Greengates medical group.

The post we have available will be based primarily at 2 of our Beverley sites but there may be a requirement to work at the other Beverley site or one of the two Cottingham Sites on rare occasions.

The Post is for 18 hours per week preferably across 3 days, 6 hours per day but can be flexible.

We are looking for an experienced practice nurse who is experienced in all aspects of routine Practice nurse work including chronic disease management. Please see the attached Job description for further details.

Main duties of the job
CONTRIBUTION TO THE PRACTICE

The post-holder will
  • Apply practice policies, standards and guidance as per staff handbook
  • Discuss with other members of the team how the policies, standards and guidelines will affect own work
  • Participate in audit where appropriate
  • Attend at least one PPG meeting per year (Patient Participation Group)
  • Attend full staff meetings when requested to do so
About us
We are a large Teaching Practice with a patient list of approximately 32,000 patients. 10 GP partners, 8 salaried GPs, 4 ANPs, 4 clinical practitioners, 6 practice nurses, 3 HCA, 3 GPA's and 1 phlebotomist. We have 3 surgeries in Beverley East Yorkshire and 2 in Cottingham also in East Yorkshire.

Job responsibilities
To provide nursing care by assisting the clinical team in all tasks as trained at the direction of the Clinical Practitioner

To allow effective communication between patients, carers, doctors and other primary care staff

To adhere to the strictest confidentiality when handling medical records and other information pertaining to patients and staff

To undertake some administrative duties when required, such as ordering of stocks when directed/indicated to do so

KEY DUTIES AND RESPONSIBILITIES
Duties may vary under the direction of the Senior Practice Nurse and Practice Manager depending on workload and staffing levels and may include the following:

Primary Nursing Duties
  • Conducting annual health checks
  • Administering travel vaccinations and promoting travel health
  • Administering child vaccinations after sufficient training
  • Long Term Condition checks and monitoring
  • Performing health checks for patients using hormonal contraception
  • Undertaking screening and diagnostic tests for sexually transmitted infections after sufficient training
  • Undertaking cervical cytology sampling after sufficient training
  • Providing wound and ulcer care
  • Undertaking suture and clip removal
  • Performing ECGs
  • Measuring blood pressure, temperature and pulse
  • Measuring height, weight, BMI and blood sugar levels
  • Ascertaining smoking status and advising on cessation, after sufficient training
  • Providing venepuncture and phlebotomy services
  • Taking peak flow readings
  • Undertaking urine testing
  • Conducting routine immunisations
  • Providing treatment during hypo/hyperglycaemic emergencies
  • Assisting in providing emergency treatment for asthma attacks
DUTIES WITHIN THE CLINICAL TEAM
  • Ensuring that specimens are passed on to the pathology service
  • Recognising changes in patient observations and taking appropriate action
  • Advising patients with regards to healthy lifestyle choices
  • Providing dietary advice for healthy eating and the recommended limits for alcohol consumption
  • Providing support and advice for smoking cessation and increasing exercise
  • Offering travel health advice and ensuring patients are referred to the appropriate member of staff
  • Administering medications on prescription if required and according to Practice protocol
  • Formulating specific wound management plans for patients
  • Acting as patient chaperone when needed
  • Being responsible for sterilising, cleaning and equipment maintenance when required
  • Applying infection control measures including safe disposal of waste materials, handling laboratory specimens, decontamination of instruments, dealing with body fluid spillages and reporting needlestick and other injuries
  • Attending regular anaphylaxis and resuscitation training and demonstrating sufficient competency in other emergencies such as stroke, haemorrhage or infarction
  • Assisting in seasonal clinics such as the covid and flu campaign and monitoring vaccine storage
  • Maintaining awareness of child health procedures and points of referral
  • Recognising indicators of child abuse and family violence, referring appropriately
  • Recognising indicators of substance abuse and addictive behaviour, referring appropriately
  • Recognising the needs of patients with depression or at risk of suicide, referring appropriately
  • Maintaining knowledge of contraceptive choices, emergency contraception and pregnancy
  • Offering advice on the causes and avoidance of sexually transmitted infections
  • Advising patients on cervical cytology and mammography screening programmes
  • Promoting breast cancer awareness, self-examination and providing accompanying literature
  • Promoting mens health including testicular self-examination
  • Assisting in the treatment of patients with newly diagnosed diabetes, supporting them in their management of the condition, after sufficient training
  • Maintaining awareness of COPD, contributing to the care of patients, demonstrating the use of inhalers and referring appropriately, after sufficient training
  • Managing patients with asthma, reviewing inhaler technique and advising on changes to treatment, after sufficient training
  • Maintaining awareness of CHD and prevention strategies, contributing to the care of patients, offering advice on management of the condition and referring appropriately, after sufficient training
  • Working with patients with CHD to minimise the risk of hypertension, angina, myocardial infarction and cerebrovascular accident, after sufficient training
  • Obtaining and documenting informed consent where appropriate
  • Clearing and re-stocking consultation rooms and couches when required
  • Ensuring stock replenishment and rotation
  • Undertaking home visits if required
OTHER DUTIES
  • Contributing to the Development Plan of the Practice,
  • Supporting the Practice clinical governance agenda and implementing changes to clinical practice according to guidelines
  • Communicating effectively with a wide range of people, whether face-to-face, over the telephone or via email
  • Maintaining high standards of cleanliness and keeping the working environment free from hazards
  • Attending and participating in regular Nursing team Practice meetings with the Lead GP and at least one Patient Participation Group meeting per year
  • Facilitating an effective learning environment and offer innovative ideas for improvement
  • Attend annual appraisal with Nurse Practitioner and plan Continuing Professional Development and training needs
  • Maintaining knowledge of developments in the NHS and keeping up-to-date with public health news
  • Maintaining familiarity with the appointment and repeat prescription system, including ensuring sufficient patient information is recorded
  • Offering administrative support, including ensuring the computer system is up-to-date with all consultations
  • Organising own work schedule and taking responsibility for prioritising workload in discussion with colleagues if indicated
  • Participating in educational activities and in-house training by sharing skills and knowledge with colleagues
  • Maintaining professional development as well as personal objectives, by taking part in organised study days and self-directed learning
  • Maintaining knowledge of and adhering to the NMC Code
  • Contribute towards maintaining health promotion information in waiting rooms, TV's and notice boards
Person Specification
Qualifications
Essential
  • Evidence of training and development within nursing, including membership of a professional body
  • Effective communication with a wide range of people
  • Good standard of education; including Maths and English qualification
  • High standard of written and verbal communication
  • Strong interpersonal skills
  • Willingness to partake in training and ongoing development
  • Understand the necessity of respecting confidentiality and diversity
  • Experience in working effectively as part of a team
  • Maintaining accuracy with regards to patient records and other correspondence
  • Motivated, polite, adaptable and enthusiastic
  • Ability to work under pressure
  • To understand the needs of patient accessibility and alternative methods of communication and to react sensitively to requests of this nature
  • Confidence to engage with children and elderly patients
  • Understanding specific patient groups and community issues
  • Experience in a front line, public-facing role within the NHS
  • Demonstrable understanding of Primary Care
Desirable
  • Good level of computer literacy, including experience in database usage
  • Previous knowledge of Systmone
