Advanced Nurse Practitioner - Health and Community Care

NHS Greater Glasgow and Clyde (NHSGGC) is one of the largest healthcare systems in the United Kingdom, employing approximately 40,000 staff across a wide range of clinical and non-clinical professions and roles. We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.

The role:

Inverclyde HSCP ANP service delivers urgent and unscheduled care on behalf of GP Practices through a dedicated home visiting model. Responding and supporting care homes residents, housebound patients and those patients too acutely unwell to visit their GP Practice; the service ensures timely, accessible care where it is most needed.

Working in close partnership with GPs and multidisciplinary teams, the service provides seamless, high-quality, and safe care within the community. ANPs collaborate with carers, care-at-home services, care home staff, and social care colleagues to deliver patient-centred support tailored to individual needs. The service manages a wide range of conditions, addressing physical and psycho-social needs of this population. ANPs carry out comprehensive clinical assessments, arrange investigations, develop treatment plans, prescribe where appropriate co-ordinate the transfer of patients to hospital when required.

The service plays a key role in health education and promotion, supporting rehabilitation, enhancing wellbeing and helping patients maintain independence in their home for as long as safely possible.

All roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.

Duration, Location, and Working Pattern:

  • This is a permanent post working 27 hours per week (various hours are available).
  • The shift pattern will be discussed with the successful candidate; however, the s hift will commence at 8.30 am.
  • You will be based at Port Glasgow Health Centre, Bay Street, Port Glasgow
  • You will be expected to work across roles and geographical boundaries, collaborating with a range of health and social care professionals throughout Inverclyde.
  • Please note the salary is pro Rata for part-time hours.

Key responsibilities:

The post holder will work across roles and geographical boundaries, delivering comprehensive clinical care as part of a multidisciplinary team. Working within a Single Point of Access model, they will respond to GP home visit requests, assessing, diagnosing, and managing a wide range of patient needs both autonomously and collaboratively.

The role includes delivering evidence-based care, providing clinical leadership and guidance to nursing and multidisciplinary colleagues, and supporting unregistered staff. The post holder will contribute to setting, implementing, and evaluating standards of nursing practice while ensuring high-quality care for residents across Inverclyde. Strong collaboration is essential, including effective liaison with GPs, out-of-hours services (NHS 24), and the Scottish Ambulance Service to ensure safe, coordinated patient care.

Knowledge, training, qualifications and/or experience required to do the job:

Advanced Nurse Practitioners (ANPs) are Master’s level clinicians with the autonomy, authority, and accountability to assess, diagnose, treat, and prescribe for patients with complex needs. Using advanced clinical judgement, they manage care independently, including referral, admission, and discharge decisions, supported by specialist knowledge and experience.

Applicants must be NMC-registered Independent/Supplementary Prescribers (V300) with a Master’s qualification in Advanced Practice and relevant clinical experience; they should have successfully completed Clinical assessment, Clinical reasoning, judgement and diagnostic decision making, Anatomy and pathophysiology, Non-medical prescribing- V300, Leading, delivering and evaluating care and Practice learning/transferable work based learning - community experience is desirable. An interest in palliative care, long-term conditions, and anticipatory care is advantageous.

A full driving licence is essential to support delivery of care to patients across community settings.

As part of the NHS Greater Glasgow & Clyde’s pre-employment checking for all appointed candidates, evidence of NMC registration will be required prior to start date. You should therefore note that candidates who are unable to provide confirmation of current NMC registration, in line with projected start date for this post, may not be shortlisted for interview. This post requires PVG Scheme Membership/ Disclosure Scotland Check. Full driving licence essential.
