As a Band 5 Physiotherapist, you'll be part of our valued Rapid Response and Intermediate Care Team based at Sheppey Community Hospital, supporting patients across Swale in both community and inpatient settings.

Main Responsibilities
As a Band 5 Physiotherapist, you will:
  • Undertake six-month rotations across Rapid Response, Intermediate Care and the Inpatient Rehabilitation Unit
  • Assess, plan, implement and evaluate physiotherapy treatment programmes for patients with a variety of complex physical health conditions
  • Complete comprehensive, holistic assessments and develop person-centred rehabilitation goals
  • Deliver evidence-based interventions designed to maximise independence and functional ability
  • Manage your own clinical caseload with support from senior colleagues as required
  • Work collaboratively with patients, carers, families and multidisciplinary colleagues to ensure coordinated care
  • Support discharge planning and facilitate smooth transitions between services
  • Maintain accurate and timely clinical records in line with professional and organisational standards
  • Participate in clinical supervision, service development initiatives, audit and quality improvement activities
  • Delegate appropriate tasks to Rehabilitation and Therapy Support Workers
  • Attend team meetings, clinical forums and in-service training sessions
  • Contribute to the continuous improvement of patient outcomes and service delivery
The Ideal Candidate
We are looking for someone who is enthusiastic about rehabilitation and community-based care and is committed to delivering excellent patient experiences.

You will have:
Essential
  • Degree or Diploma in Physiotherapy
  • HCPC registration as a Physiotherapist (or be awaiting registration as a final-year student)
  • Strong assessment, treatment and clinical reasoning skills
  • Excellent communication and interpersonal abilities
  • Effective organisational and prioritisation skills
  • Good IT skills, including Microsoft Office applications
  • Ability to work independently and as part of a multidisciplinary team
  • Full UK driving licence and access to a vehicle for work purposes
Desirable
  • Membership of the Chartered Society of Physiotherapy (CSP)
  • Previous experience in community, rehabilitation, neurology, musculoskeletal or orthopaedic settings
  • Understanding of clinical governance, audit and quality improvement
  • Evidence of continuing professional development

Qualifications
Essential
  • Degree or Diploma in Physiotherapy, HCPC registration as a Physiotherapist, strong assessment, treatment and clinical reasoning skills, excellent communication and interpersonal abilities, effective organisational and prioritisation skills, good IT skills, ability to work independently and as part of a multidisciplinary team, full UK driving licence and access to a vehicle for work purposes.
