Health and Wellbeing Practitioner - HMP Send

We are looking for a Health and Wellbeing Practitioner to join The Forward Trust at HMP Send, Surrey, working full-time, 37.5 hours per week.
In this role, you'll deliver clinical and psychosocial substance misuse services within a trauma-informed, recovery-focused framework, supporting female service users with substance misuse and low-level mental health needs. You'll help them build healthier futures both inside the prison and as they transition back into the community.
What you'll be doing:
* Manage your own caseload, delivering 1:1 and group interventions tailored to individual needs
* Conduct assessments, co-produce care plans, and provide harm reduction advice
* Facilitate release planning and through-care support for service users returning to the community
* Work collaboratively with multi-disciplinary teams across mental health, healthcare, and the prison
* Ensure accurate, timely recording of outcomes and data to track the impact of your work About you: * Compassionate, proactive, and solution-focused

* Ready to work weekends on a rota basis
