* Undertake comprehensive risk assessments and provide detailed reports.
* Conduct face-to-face visits with families to gain insights and deliver effective care plans.
* Document all records and assessments using the Mosaic system.
* Arrange the provision of essential equipment as required.
* Initiate major adaptation referrals for those appropriate cases.
* Secure manager sign-off and ensure case closures are handled efficiently.
* Work independently while maintaining a high standard of professional care. Requirements * Degree in Occupational Therapy, supported by HCPC registration.
* Significant experience in Children's Occupational Therapy and Social Care.
* Proven ability to work autonomously whilst managing complex cases.
* Competency in conducting assessments, making equipment recommendations, and processing adaptation referrals.
* Exceptional report writing and case management skills. Desirable Requirements * Familiarity with the Mosaic system.
* Experience with Local Authorities and waiting list clearance projects.
* Knowledge in moving and handling.
