Our Intermediate Care Service supports adults for up to six weeks, helping them regain skills, confidence and independence after a period of ill health or hospital stay. We work to a Home First approach, helping people remain at home for longer, avoid unnecessary hospital admissions and return home safely and quickly when they do need hospital care.
We are looking for caring, compassionate and motivated individuals to join us as Intermediate Care Workers. You will work predominantly in people’s homes, supporting them to achieve their personal outcomes, remain independent and live life to the fullest.
The ability to travel across the Craven Area is essential.
What you’ll be doing
Support independence & wellbeing
- Provide person‑centred care that promotes dignity, respect and choice.
- Support adults with personal care, daily living tasks, meal preparation and household activities.
- Encourage people to access community facilities and universal services.
- Help individuals regain mobility and confidence through reablement-focused activities.
Deliver safe, high-quality care
- Work in line with the Care Act, Mental Capacity Act and CQC Fundamental Standards.
- Use PPE and comply with Infection Prevention and Control procedures.
- Support individuals with medication, following training and competency assessment.
- Undertake delegated health tasks and administer emergency first aid (training provided).
- Assist with safe moving and handling, using appropriate equipment.
Work collaboratively
- Be part of a multi-disciplinary team including Health, Social Care and voluntary sector colleagues.
- Communicate effectively with individuals, families, carers and partner professionals.
- Use communication aids where appropriate such as picture cards, Makaton or dementia tools.
Record keeping & technology
- Maintain accurate, professional care records.
- Use electronic scheduling, care recording systems and technology enabled care tools.
- Contribute to support plans and provide regular progress updates to your manager.
Safeguarding & risk management
- Promote the welfare of vulnerable adults and identify any safeguarding concerns.
- Support people to manage acceptable levels of risk in their daily lives.
- Identify falls or environmental risks and escalate when necessary.
We’re looking for someone who is compassionate, patient and committed to supporting others, and who can work confidently and safely as a lone worker in the community. You will need to be competent in written and spoken English and willing to undertake relevant training, including the NCFE Level 2 Diploma in Care if you’re new to the sector. Flexibility to work shifts across a 7‑ day service, including evenings and weekends, is essential. While experience in care is welcome, it isn ’ t essential, what matters most is your values, attitude and willingness to learn.
