About the Role:
As a Social Worker within our Adults with Learning Disabilities team, you will be responsible for:
- Conducting comprehensive assessments of individual needs, strengths, and risks, in line with relevant legislation and guidance.
- Developing and implementing person-centred support plans in collaboration with individuals, their carers, and other professionals.
- Providing direct support and interventions to individuals, including advocacy, emotional support, and practical assistance.
- Working collaboratively with a range of partner agencies, including health professionals, housing providers, and voluntary organisations, to ensure coordinated and effective support.
- Safeguarding vulnerable adults in accordance with local policies and procedures.
- Monitoring and reviewing support plans to ensure they remain relevant and meet individual needs.
- Maintaining accurate and up-to-date records in line with professional standards and organisational requirements.
- Contributing to the ongoing development of the service through participation in team meetings, training, and service improvement initiatives.
Qualifications and Skills:
- A recognised Social Work qualification (e.g., BA/MA in Social Work, DipSW).
- Registration with the Scottish Social Services Council (SSSC) as a Social Worker.
- A full driving licence and access to a vehicle.
