An exciting opportunity has arisen to join the Care Brokerage Team which sits within the Urgent and Emergency Care and New Models of Care portfolio. The Brokerage Officer position is a FTE (37 hours per week) post, operating Monday to Friday.

About the Role:
- To manage referrals for Adult Social Care and Health services via a single point of access, ensuring referrals include all relevant information to inform providers accurately. This also involves discussions with clients, carers and partner organisations.
- Develop and maintain relationships with providers and become familiar with services in order to match clients to appropriate services.
- Develop and maintain relationships with referral sources and to raise awareness of referral processes and available support and to act as the conduit between referral sources and providers.
- Identify and make referrals to the most appropriate providers based on the information gathered from the referral.
- Monitor and assess the progress of referrals, liaising with providers, responding to calls, emails and queries and ensuring all systems are updated in a timely manner.
- To facilitate timely hospital discharges to support system flow.

The successful candidate will demonstrate:
• Experience of working within the Care Sector
• Experience of working with Health would be an advantage.
• The ability to work under minimal supervision in a fast paced and high-pressure environment.
• The ability to be flexible to the demands of the service.
• Excellent relationship management skills
• Excellent IT skills
