An exciting opportunity has arisen within the Renal Department to join the small team of Chronic Kidney Disease Nurses. We are a dynamic team, providing expert, specialist renal care within National recommendations and guidelines. This expertise is reflected in the consistent high ranking of CIC/WCH in the national Patient Report Experience Measure (PREM).

The successful candidate will be expected to work autonomously in the outpatient/clinic and community settings. They will have excellent communication skills when discussing patient's treatment, conditions and identifying problems within the complexities of renal disease processes. We also provide immediate and ongoing care of the transplant patient and support the live kidney donor programme.

The CKD service is Monday - Friday.

Main duties of the job
The role of the CKD specialist nurse is to provide multidisciplinary treatment and support for patients who are suffering from renal impairment, including those who progress to end stage renal disease requiring dialysis as a life supporting therapy.

You will work as part of a wider multidisciplinary CKD team, supporting your colleagues to deliver effective integrated health and social care to patients, communicating complex and sensitive information about patients, families and carers.

You will communicate across organisational boundaries and work as required with other bodies in the statutory, third and voluntary sectors to ensure the health and social care needs of the individual are met appropriately and safely. This will include receiving and making referrals, requesting medical assistance and joint working to develop services.

Qualifications
Essential

• NMC Registered Nurse
• Degree or equivalent
• Experience at senior level
• Coaching and mentoring register or ENB 136/998 or equivalent
• Evidence of leadership or management development
• Further relevant postgraduate development at level 6 or senior clinical experience

Desirable

• Evidence of Clinical Practice development
• Counselling qualification or equivalent

Knowledge & Experience
Essential

• Relevant experience in the care of renal patients and recent experience in both peritoneal dialysis and haemodialysis, as well as recent experience at senior staff nurse level
• Recent experience of managing a caseload of patients receiving renal replacement therapy
• Experience in the development of a nurse led service
• Budgetary management skills
• Roster management skills and knowledge of same
• Dealing with critical incidents/complaints
• Teaching experience in the healthcare setting
• Evidence of being able to work as an autonomous practitioner
• Previous experience of clinical supervision
• Knowledge of renal nursing and an in depth knowledge of the recent advances in management of patients requiring renal replacement therapy and those being prepared for dialysis
• Able to act as a clinical supervisor
• Knowledge of quality markers and government guidelines relating to the management of renal patients
