Legal right to Work: In accordance with Home Office guidance, successful candidates must evidence their right to work in the UK before commencement of employment. 

Medical Clearance:  Pre-placement health assessment (PPHA)  

Casual Car Documentation:  The successful candidate will be a designated Casual Car User which means that car journeys may be necessary on an occasional basis, but are not essential to the role. If you do drive, you must always hold a full valid driving licence and provide your own car for use at work, as well as provide all the appropriate documentation . You will receive an allowance per mile of business use.

DBS (Disclosure and Barring Service) Clearance: This post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended in 2013). Appointable candidates will be required to apply for an Enhanced Disclosure and Barring Service (DBS) check. Manchester City Council, as part of Manchester Local Care Organisation (MLCO), is transforming how health and social care services are delivered across the city through our Integrated Neighbourhood Teams (INTs).

We are seeking an Assistant Reablement Team Leader to support the delivery of high-quality, person-centred reablement services. This role will combine hands-on practice with day-to-day operational support, helping to ensure individuals with complex needs are supported to regain independence and achieve positive outcomes within their communities.

You will work closely with the Team Leader to coordinate service delivery, support staff, and maintain high standards of care and performance.

Key Responsibilities

Service Delivery

Support the delivery of high-quality reablement services within the neighbourhood

Carry out and oversee medication assessments and medication audits 

Ensure service users receive person-centred, outcome-focused care

Team Support and Supervision

Assist the Team Leader with day-to-day coordination of the service

Provide guidance, informal supervision, and support to team members

Support staff to deliver effective, safe, and responsive care

Contribute to induction, training, and development of staff

Carry out staff observations and staff medication observations

Partnership Working

Work collaboratively with health professionals, social workers, and partner agencies

Support effective multi-disciplinary working within the INT model

Enable access to community and voluntary sector services

Performance and Quality

Monitor and support the team to meet service targets and outcomes

Ensure accurate and timely record keeping and data collection

Contribute to quality assurance and service improvement activities

Additional Responsibilities

Promote equality, diversity, and inclusion in all aspects of service delivery

Maintain a commitment to continuous professional development

Contribute to the ongoing development of integrated neighbourhood working

About the Candidate

We are looking for a motivated and experienced individual who:

• Has demonstrable experience within reablement, health, or adult social care services
• Understands and applies person-centred and strengths-based approaches in practice
• Is able to support, supervise, and motivate staff within a multidisciplinary team
• Has sound knowledge and experience of risk assessment, care planning, and professional decision-making
• Demonstrates excellent communication, organisational, and problem-solving skills
• Is adaptable, resilient, and able to work effectively in a busy, fast-paced environment
• Is committed to delivering high-quality services that promote independence, dignity, and positive outcomes
