JOB TITLE: Specialty Doctor Elderly Care/Intermediate Care – Mountnessing Court (Billericay) CONTRACT: 6 Programmed Activities Substantive Flexible working available – including job share, part-time, remote sessions considered BASE: Mountnessing Court, 240 Mountnessing Road, Billericay, CM12 0EH Post and Specialty: This is a well-established post but is vacant due to the previous doctor leaving the organisation. This is funded as 6 Programmed Activities (6 PAs) post with 5 Direct Clinical Contact activities (DCC) and 1 Supporting Professional Activities (SPAs). This is a substantive, part time (3 days) post and we are also open to consider flexible working, job share or fixed term if preferred. We are kind We are respectful We work with our communities This post is for a Specialty Doctor position in one of our three Intermediate Care Wards, at Mountnessing Court, 240 Mountnessing Road, Billericay, CM12 0EH Rationale for post: This is a well-established post soon to be vacant due to the previous doctor leaving the organisation. A lot of transformation has taken place already and continues to take place within Intermediate Care Services in Essex. This post is currently vacant, and we wish to recruit to this post as soon as possible. This is a replacement, substantive, part time post – 6 Programmed Activities (PAs) post with 5 Direct Clinical Contact activities (DCC) and 1 Supporting Professional Activities (SPAs). The successful candidate will work as part of an experienced multidisciplinary team providing care to adults and older people admitted to the Community Hospital Inpatient ward for Rehabilitation and discharge planning. The post holder will be provided with clinical supervision and support from existing consultants. They will gain experience in managing complex medical and frail elderly patients requiring step down care from acute hospitals across the Mid and South Essex locality. 1.2 Main responsibilities Main responsibilities: • To be responsible for the assessment and clerking of new admissions, managing ongoing medical issues and completing electronic discharge letters • To assist with twice weekly consultant ward rounds and attend once weekly multidisciplinary team meetings for efficient discharge planning. • To provide clinical response to any emergency clinical issues that might arise. • To have organisational knowledge relating to Trust protocols and procedures and adhere to them. • To be responsible for providing accurate records of information using the Systm One electronic medical record as required by the Trust for audit purposes. To ensure effective risk management at team level by accident/incident reporting, assessing, and controlling risk and ensuring residual risks are added to the Trust’s risk register. Clinical Skills: To act as an autonomous, registered practitioner who is legally and professionally accountable for own unsupervised actions guided by the professional code of conduct and Trust guidelines and protocols. The post holder will have full registration with a licence to practice on the General Medical Council (GMC). Further professional knowledge will have been gained through accredited courses, workshops, study, and in-house training programmes. To be responsible, and accountable, for service delivery to clients/patients. To be able to assess and develop care plans to meet the complex needs of patients with a variety of conditions. This includes chronic, acute, and palliative care within own competencies, recognising own limitations and seeking advice when necessary. This will include continuously evaluating and acting on outcomes. To be able to initiate referrals to other health professional specialist services and agencies. To provide patients and relatives with information and education thus ensuring they have meaningful choices that promote dignity, independence, and quality of life. To ensure practice is supported by research, evidence-based practice, literature, and peer review.
