Children's Learning Disability Nurse

Every child deserves the opportunity to achieve their full potential. As part of our Children's Community Learning Disability Health Service, you will play a vital role in improving the health, wellbeing and life outcomes of children and young people with learning disabilities across Wiltshire.

This is a rewarding opportunity to work closely with children, families, carers, schools and partner organisations, providing specialist support that makes a meaningful difference. Through assessment, care planning and collaborative working, you will help ensure children and young people receive the right care, in the right place, at the right time.

About the Role
We are seeking a compassionate and motivated professional to join our Children's Community Learning Disability Health Service. The post holder will contribute to the continued development of a flexible, responsive and person-centred service for children and young people with learning disabilities throughout Wiltshire.

Working in partnership with families, carers, education providers, health professionals and statutory and voluntary agencies, you will support the delivery of coordinated care that promotes positive health outcomes and enables children and young people to reach their individual potential.

You will undertake assessments, contribute to healthcare planning and provide specialist advice and support in a variety of settings, including family homes, educational environments and community health premises.

Location: Derby Court, Epsom Square, White Horse Business Park, Trowbridge, Wiltshire, BA14 0XG
Hours: 37.5 hours per week

Key Responsibilities
  • Manageadedicatedcaseloadofchildrenandyoungpeoplewithlearningdisabilities,deliveringhigh-quality,person-centredcareandsupport.
  • Undertakeholisticassessments,developandreviewevidence-basedcareplans,andcoordinateappropriatereferralstootherserviceswhererequired.
  • Workinpartnershipwithchildren,youngpeople,familiesandcarerstoensureindividualneedsareunderstood,supportedandreflectedincareplanning.
  • Providespecialisthealthadvice,interventionsandhealtheducationtopromotewellbeing,independenceandimprovedhealthoutcomes.
  • Collaborateeffectivelywithmultidisciplinaryteamsandexternalpartners,includingschools,earlyyearssettings,socialcareandvoluntaryorganisations,todelivercoordinatedcare.
  • Advocateforchildrenandyoungpeople,ensuringtheirvoiceisheardandtheirdignity,choice,safetyandwellbeingarealwaysupheld.

Person Specification
Essential
  • Registered Learning Disability Nurse with current NMC registration.
  • Postgraduate diploma or equivalent level of knowledge and experience in learning disability nursing.
  • Teaching and assessing qualification, or experience supporting learning and development within clinical practice.
  • Experience of working with children and young people with learning disabilities in a community setting.
  • Sound knowledge and experience of safeguarding children and vulnerable young people.
  • Experience of managing a caseload, including assessment, care planning, intervention and review.
  • Ability to develop and deliver person-centred, evidence-based care plans that meet complex health needs.
  • Strong communication and relationship-building skills, with the ability to engage effectively with children, families, carers and multidisciplinary professionals.
  • Knowledge of positive behaviour support, mental health, transition planning, sleep management, toileting, independence skills and supporting children with complex needs.
  • Competent in the use of clinical and IT systems, including SystmOne, Microsoft Word and Excel.
  • Evidence of ongoing professional development and commitment to maintaining clinical competencies.
  • Flexible, adaptable and able to work independently, prioritising workload effectively across a varied caseload.
  • Full driving licence and ability to travel throughout Wiltshire to meet service needs.
Desirable
  • 2-3 years' post-registration experience within a learning disability, children's or community healthcare setting.
  • Postgraduate Community Nursing Qualification or equivalent.
  • Degree-level qualification or equivalent experience.
  • Experience of leading, supervising or supporting a team.
  • Experience of delivering training and education to families, carers or healthcare professionals.
  • Knowledge of current legislation, policy and best practice relating to children and young people with learning disabilities.
