Are you a dynamic and motivated individual with strong leadership skills? If so the Bolton Home Based Treatment Team is seeking to recruit a Senior Urgent Response Practitioner at Band 7 to provide Clinical Leadership working alongside the Team Manager supporting service delivery and development. The Home Based Treatment Team is an established service and comprises a dynamic and welcoming team of psychiatrists, senior practitioners, practitioners as well as a psychological therapist. The team has senior level input into the local and GM strategic vision for Home Treatment services. The HBT operates 24/7 to provide responsive mental health care in the community setting, delivered by experienced and established multidisciplinary teams. We aim to provide an alternate to hospital admission and care for service users in the least restrictive way. As a Trust, we are committed to our services delivering models that are clinically led, operationally partnered and academically informed. The Senior Practitioner will develop close links with the Inpatient Wards to promote HBTT interventions with service users with the view to supporting earlier discharge from the inpatient wards aiming to reunite service user with their families / carer whilst also maintaining HBTT support in the community. Main duties of the job
Provide clinical leadership, line management supervision, support and consultation to HBT practitioners, in line with the clinical pathway as outlined within the operational procedure. Manage and evaluate a caseload in line with the clinical pathway, as outlined within the operational procedure. This will include:

- Build and maintain good working relationships with other agencies both in and out of the Bolton area, to ensure the best care delivery for the service user and their carers and/or families.
