Description:
Part time Performer Dentist required for 2 days per week, in a well established, practice near Blackburn, Lancashire.
• Tues & Weds
• 2200-3000 UDAs - Up to £15 per UDA
• EXCELLENT PRIVATE POTENTIAL
• 50% split on good private earnings
• 50% split on labs
• Fully computerised surgeries - 8 surgery practice
• iTero Scanner
• OPG machine
• Digital x-rays
• Rotary Endo
• Therapist on-site
• Experienced and qualified nursing team
• PM on-site with a full support team
• Free parking on-site
• 15 minutes to Blackburn, 30 minutes to Preston and Bury, 50 minutes to Manchester.
• Available Oct 26, start date negotiable

Compliance requirements:- Full NHS Performer Number
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
