What you will do

* Provide specialist clinical pharmacy input, including medicines reconciliation, clinical screening, and therapeutic drug monitoring.
* Review and optimise medication for patients with acute and long-term conditions, ensuring safe and effective prescribing.
* Collaborate with consultants, nursing staff, and other members of the multidisciplinary team to support high-quality patient care.
* Promote medicines optimisation and ensure that prescribing practices comply with relevant clinical guidelines, policies, and governance standards.
* Offer professional support and guidance to pharmacy colleagues, including pharmacists and pharmacy technicians.

Requirements

* Current GPhC registration as a Pharmacist.
* Strong post-registration clinical pharmacy experience.
* Previous hospital pharmacy experience is highly desirable.
* Excellent knowledge of medicines optimisation, clinical screening, and medicines reconciliation.
* Ability to work confidently and effectively within a multidisciplinary clinical environment.
