- Use clinical reasoning to assess functional ability and recommend interventions, equipment, and adaptations.
- Contribute to the prevention of escalation in need through responsive interventions.
- Work jointly with other professionals to coordinate effective support.
- Monitor progress and adjust interventions based on changes in circumstances.
- Maintain accurate and timely case records in line with professional standards.
- Apply safeguarding principles to protect adults from harm. Requirements: - HCPC Registration is required.
