This role is ideal for a registered nurse/newly qualified who are enthusiastic about delivering high-quality, person-centred care to patients in their own homes and supporting people to remain safe and independent within the community. You will work as part of a supportive and integrated team, caring for housebound patients with a range of nursing needs including wound care, palliative care, catheter care and long-term condition management. We are looking for someone who communicates effectively, works well both independently and as part of a team, and is committed to continuous professional development. The post holder plays a vital role in supporting the District Nursing team through the management of an assigned caseload. Key responsibilities encompass the assessment, planning, and evaluation of patient care needs. The incumbent works closely with the District Nurse to facilitate effective communication regarding patient care, maintain up-to-date care plans, and ensure seamless continuity of services, especially during reviews of complex care arrangements. Additional duties include contributing to mentoring, educational, and assessment activities. Collaboration with healthcare professionals and external agencies is integral to providing efficient and high-quality service delivery. About us

- Perform comprehensive assessment of patient nursing needs as the named nurse.
- Plan, assess and evaluate programmes of care according to changing health needs under the direction of the District Nurse.
- To support the team leader in the co-ordination of complex and continuing care packages.
- Maintain accurate records in accordance with NMC.
- Attend a multi-disciplinary case conference as the named nurse if appropriate.
- To communicate and collaborate with other members of the PHCT, voluntary and statutory organisations to support individual patient care.
- To establish and maintain effective communication with patients, relatives and carers in complex and potentially challenging situations.
- To competently perform clinical skills related to the post.
- To be professionally responsible for a delegated caseload under the supervision of the District Nurse.
