We have an outstanding opportunity for a Band 5 Community Nurse to join the vibrant Home First South Team within Community General Nursing in Fareham. This role, with the option for Full or Part Time hours, offers a 3 month Block Booking and operating within the hours of 09:00 - 17:00. * Facilitate early supported discharge and admission avoidance through proactive care management.
* Work collaboratively within a multidisciplinary team, liaising with health, social care, and voluntary sector partners to coordinate care. Qualifications: * Registered Nurse (Adult) with an NMC Active PIN
* Relevant pre-registration nursing degree or diploma Registration & Compliance: * Current registration with the Nursing and Midwifery Council (NMC)
* Knowledge of NHS clinical governance frameworks and CQC standards
* Understanding of the Mental Capacity Act and safeguarding principles
* Commitment to Continuing Professional Development (CPD) and revalidation requirements Experience: * Post-registration experience in a community, acute or integrated care setting
* Experience supporting patients with complex health needs in the community
* Exposure to discharge planning or admission avoidance services is desirable
