Job Title: Locum Social Services Occupational Therapist - Community Adults

- Manage a potentially complex caseload with support, prioritising work, and allocating cases to provide Occupational Therapy to adults with care and support needs in their homes.
- Conduct asset-based, face-to-face assessments with adults and their carers, addressing functional needs and care issues while considering available resources and digital solutions.
- Tackle urgent requests to resolve moving and handling issues effectively.
- Develop, implement and review reablement plans, setting goals and outcomes with adults and their carers.
- Explore and implement various solutions including care support, techniques, equipment, and adaptations to meet customer needs.
- Complete recommendations and applications for adaptations and equipment for local agencies and councils.
- Assess specialist equipment with suppliers considering best value solutions, and follow up on them as needed.
- Collaborate with community and acute health services, along with other partner agencies, both statutory and voluntary.
- Maintain accurate records, assessments, notes, and recommendations, ensuring confidentiality on databases.
- Interpret Care Act Eligibility and other relevant legislation concerning Occupational Therapy issues in Adult Social Care.
- Contribute to team systems effectively for urgent and standard work.
- Participate actively in supervision, team meetings, and professional development of self and colleagues. Requirements of the Occupational Therapist:
- Degree or equivalent in Occupational Therapy.
- Recent UK experience working as an Occupational Therapist, particularly within the NHS.
