Key Responsibilities Face to face, telephone and online consultations Diagnosis and management of acute and chronic conditions Prescribing of medications and appropriate treatments Undertake home visits as and when required Reviewing of pathology and diagnostic results Referral of patients to secondary care and specialist services Maintain accurate and contemporaneous patient records Promotion of health education and disease prevention Support management of long-term conditions including diabetes, asthma and hypertension 2 On-Call sessions per week Safeguarding & Governance Adhere to safeguarding procedures for children and vulnerable adults Participate in clinical governance, audits and quality improvement activities Maintain confidentiality and comply with GDPR and information governance standards Follow infection prevention and control procedures Administrative Responsibilities Completion of clinical documentation promptly Processing of prescriptions, referrals and medical certificates Manage workflow tasks and correspondence An element of Non-NHS work, including insurance/medical reports
