What you will do:

* Provide high-quality Occupational Therapy assessments and interventions within an acute medicine ward setting.

* Assess and treat acute inpatients, supporting their rehabilitation, recovery, and functional independence.

* Facilitate effective discharge planning to promote safe and timely patient discharges.

* Work collaboratively with multidisciplinary teams to deliver coordinated, patient-centred care.

* Manage and prioritise a varied patient caseload efficiently and effectively.

* Assess equipment, environmental, and support needs to enable safe discharge and prevent readmission.

* Develop person-centred treatment plans tailored to individual patient goals and needs.

* Provide advice, education, and support to patients, families, and carers.

* Maintain accurate and comprehensive clinical records in accordance with professional and organisational standards.

* Contribute to service improvement initiatives and the ongoing development of Occupational Therapy services.

Requirements:

* Degree or equivalent qualification in Occupational Therapy.

* Current HCPC registration.

* Previous experience working within acute inpatient settings.

* Experience with discharge planning and hospital flow processes.

* Minimum of six months' recent UK NHS experience.

* Strong assessment, rehabilitation, and clinical reasoning skills.

* Excellent communication and multidisciplinary team-working abilities.

* Ability to manage a busy and varied caseload effectively.
