We are looking for a compassionate and motivated

Community Staff Nurse to join our Tunbridge Wells Community Nursing Team on a 28-hour per week basis. This rewarding role involves visiting housebound patients aged 18 and over in their own homes, delivering personalised nursing care to support those with complex and long-term conditions to remain safe, well and independent in their chosen environment. Working as part of a multidisciplinary team aligned to Primary Care Networks, you will undertake holistic assessments, plan and deliver care, provide health promotion and support self-management, and carry out a range of clinical interventions including wound care, medication administration, long-term condition management, and palliative and end-of-life care.

• To support in the supervision and development of all junior staff including Health Care Support Workers and students.
• Assessment, planning, implementation and evaluation of holistic care linked to packages of prescribed care.
• Development of integrated care programmes in partnership with the patient
• To Actively work as a member of the multi-professional team to provide high quality care to patients.
• To undertake a range of competent clinical interventions to support the outcomes of both individual and group therapy including catheter care, administration of medication, wound care and end of life care
• Manage a caseload of patients on a daily basis consisting of patients with complex needs associated within a specific speciality
