Liquid Personnel is currently recruiting for an Adults Social Worker to join a busy Hospital Discharge Team based in Rochdale.

Your main responsibilities as a Social Worker:
- Undertaking section 2s and 5s.
- Facilitating hospital discharge
- Carrying out assessments in different wards.
- Managing a busy caseload within an acute hospital setting.
- Working closely with health colleagues.
- Ensuring adults in hospital are discharged in a timely and appropriate manner.
- Attending multi-disciplinary meetings.
- Mental capacity assessments leading to best interest decisions.
- Advising patients and families who do not meet the eligibility criteria and redirect them to other agencies.
What we are looking for in a Social Worker:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK licence
- Post-qualified experience
- Degree level or equivalent in Social Work
