We have an exciting opportunity to join our innovative Community-based Frailty Hospital at Home Team in the London Borough of Barnet. The team is transforming healthcare for people living with frailty by delivering high-quality care in patients' own homes, helping to reduce unnecessary hospital admissions and support timely discharge. Working within the Medicines Management Team, you will provide specialist pharmaceutical care as an Advanced Clinical Pharmacist for patients with complex needs. You will manage your own clinical caseload, undertake comprehensive medication reviews in patients' homes, and support the safe and effective use of medicines as part of a multidisciplinary team. You will also provide expert pharmaceutical advice to health and social care professionals and contribute to the ongoing evaluation and development of the service to improve patient outcomes. The post is based in Barnet and involves travelling across the borough, including lone working in the community, so access to a car is essential. As an Advanced Clinical Pharmacist within the Community Frailty Hospital at Home Team, you will play a key role in delivering expert medicines optimisation for patients with complex health needs, supporting safe, effective and person-centred care in the community. To be successful in this role, you will:
- Have significant post-registration experience in hospital pharmacy, particularly in the care of older people and frailty.
- Hold a postgraduate qualification in pharmacy to at least Diploma level.
- Demonstrate sound knowledge of prescribing and medicines management across both primary and secondary care settings.
- Apply excellent clinical decision-making, problem-solving and professional judgement when managing complex patients.
- Work effectively as part of a multidisciplinary team while managing your own clinical caseload.
- Build positive working relationships with a wide range of health and social care professionals, including GPs, hospital clinicians, district nurses, community pharmacists, care coordinators, carers and families.
- Demonstrate compassion, flexibility, excellent communication skills and effective time management in delivering high-quality patient care.
