Assess, diagnose and manage patients through face-to-face and telephone consultations.
Prescribe medications and review treatment plans in accordance with evidence-based guidelines.
Refer patients appropriately to secondary care and other community services.
Undertake routine chronic disease management, preventative care and health promotion.
Maintain accurate and contemporaneous clinical records.
Work collaboratively with the wider multidisciplinary team to ensure high-quality patient-centred care.
Recognise and manage urgent and potentially serious presentations, escalating care where appropriate.
Contribute to clinical governance, audits, significant event reviews and practice quality improvement.
