Individual Assessment and Commissioning Team Lead

Job summary
We are seeking experienced and motivated registered nurses to join our Individual Assessment & Commissioning (IAC) Service as Band 7 Team Leads.

We have vacancies in:
  • Cheshire (East & West Cheshire)
  • Mental Health/Learning Disabilities (MH/LD)
This is an exciting opportunity to lead and support a dedicated team responsible for assessing, commissioning and reviewing care for individuals with complex health needs. Working across Continuing Healthcare (CHC), funded nursing care, Section 117 Aftercare and complex commissioning pathways, you will play a key role in ensuring people receive high-quality, personalised care and support.

As a Team Lead, you will provide clinical leadership, manage complex cases, support service development, and work collaboratively with health, social care and provider partners across the system.

Main duties of the job
What we're looking for
  • Registered Nurse with current NMC registration
  • Significant experience of Continuing Healthcare/ Mental Health/Learning Disabilities and/or Complex Case Management
  • Strong leadership and decision-making skills
  • Experience managing complex assessments, reviews and multi-agency working
  • A passion for delivering person-centred care and improving outcomes

Job responsibilities
  • To provide specialist nursing assessment, procurement, case management and review for adults and young people in transition to adult services presenting with complex long-term care needs, who are eligible or potentially eligible for NHS Continuing Health Care or Funded Nursing Care or section 117 aftercare services or Joint Funded.
  • To provide assurance around quality of commissioned care packages, reporting safeguarding concerns in accordance with agreed interagency reporting procedures.
  • To carry a case load of the most complex client group, including adults who have experienced an acquired brain injury and /or require medical ventilation or tracheotomy care
  • Provide case management and review for service users with a mental health need working in partnership with the Local Authorities
  • To ensure safe discharge and transfer of individuals from highly supportive and acute care environments to community placements avoiding out of area placements.
  • To have an understanding of the Mental Capacity Act and Deprivation of Liberty Safeguards; completing relevant documentation for Court as appropriate
  • The post holder will deputise for the Deputy Head of Individual Assessment and Commissioning as required.
Qualifications
Essential
  • Educated to post-graduate level in relevant subject or equivalent level of experience of working at a similar level in specialist area
  • Current NMC Registration.
  • Evidence of post qualifying and continuing professional development
Knowledge
Essential
  • Knowledge and expertise of National Framework for NHS Continuing Healthcare and NHS-funded Nursing care.
  • Understanding of deprivation of liberty safeguarding processes.
  • Understanding of the public sector
  • Working knowledge of Microsoft Office with intermediate keyboard skills
Experience
Essential
  • Experience Completing complex case reviews and reports with recommendations
  • Significant experience of mental Health and/or Learning Disability and /or Complex clients experience
  • Experience of managing risks and reporting
  • Experience of Mental Health and/or Learning disability and/or Complex cases Commissioning
  • Experience of managing and motivating a team and reviewing performance of the individuals. Skills for managing relationships with a range of different stakeholders.
  • Experience of setting up and implementing internal processes and procedures
  • Experience of creating and giving presentations to a varied group of internal and external stakeholders
Skills
Essential
  • Ability to analyse very complex issues where material is conflicting and drawn from multiple sources.
  • Demonstrated capability to act upon incomplete information, using experience to make inferences and decision making
  • Demonstrated capability to plan over short, medium, and long-term timeframes and adjust plans and resource requirements accordingly
  • Numerate and able to understand complex financial issues combined with deep analytical skills
