There may be, on occasion, a requirement to carry out other tasks depending on workload and staffing levels.
- Undertake structured medication reviews and proactively manage polypharmacy to optimise patient outcomes.
- Support patients with long-term conditions through medication optimisation and evidence-based therapeutic review.
- Provide dedicated clinical pharmacy support to care homes, including undertaking structured medication reviews for care home residents.
- Respond to medicines-related queries from care home staff and support safe medicines management within care home settings.
- Ensure medicines are safe, appropriate, and effective, identifying and reducing medicines-related risks.
- Provide expert, evidence-based medicines advice to GPs, nurses, and other members of the multidisciplinary team.
- Respond to medication-related queries from practice staff and patients in a timely and professional manner.
- Reconcile medicines following hospital discharge and other care transitions to ensure continuity and safety.
- Contribute to medicines safety initiatives, clinical audit, and quality improvement activity.
- Supervise and support pharmacy technicians as part of the practice pharmacy team.
- Maintain accurate and contemporaneous clinical records using EMIS and other IT systems in line with practice policy and professional standards.
- Maintain patient confidentiality at all times in accordance with information governance requirements.
- Engage in continuing professional development to maintain and enhance clinical knowledge and competence.
Experience
Essential
- Strong clinical and therapeutic knowledge with up-to-date understanding of NICE and relevant national and local guidelines.
- Experience in medicines optimisation and structured medication reviews.
- Understanding of general practice and primary care prescribing processes.
- Ability to manage polypharmacy and long-term condition medication management.
- Excellent communication and interpersonal skills (written and verbal).
- Good IT skills and confidence using clinical systems.
- Ability to work autonomously and as part of a multidisciplinary team.
- High attention to detail with the ability to prioritise workload and manage competing demands.
- Commitment to confidentiality, professionalism and continuous professional development.
- Enhanced Disclosure Barring Service (DBS) check.
- Full UK driving licence with access to a vehicle.
- Experience working in general practice or primary care.
- Experience using EMIS and/or Docman (or similar clinical systems).
- Experience supervising or supporting pharmacy technicians.
- Experience contributing to clinical audit or quality improvement activity.
Essential
- Masters degree in pharmacy (MPharm) or equivalent.
- Registered Pharmacist with the General Pharmaceutical Council (GPhC).
- Independent Prescriber qualification.
- CPPE Primary Care Pharmacy Education Pathway (or working towards completion).
- Postgraduate diploma in Clinical Pharmacy or equivalent postgraduate qualification/experience.
