Job summary
Do you have the skills to work in community physical health? Are you interested in working for a dynamic and innovated trust who provide person centred health care to our local community patients? We'd love you to join our teams. We have an exciting opportunity for a Band 5 enthusiastic, motivated, and keen nurse to join our established multidisciplinary Care Community Team at Chester South to support and deliver a range of quality nursing services to our patients. Your skills will be really valued by our teams and patients alike. If you are new to community health, this is a great opportunity to gain experience in this unique field of nursing. We will do our best to welcome you and help you to settle in as a member of the team. Your contribution, time, skills and caring will make a huge difference to the patients we care for. Main duties of the job
Our eight care community teams are based in various locations within Chester, Ellesmere Port, Neston, Frodsham and the Rural locality covering the geographic area of West Cheshire. It's important that you can drive and have access to your own vehicle for work. But, of course, mileage expenses are reimbursed in addition to your hourly rate. As a Community Staff Nurse the successful candidates will undertake duties involving the delivery of high-quality nursing services and the holistic assessments of patients ensuring a person-centred approach. You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings. About us
Cheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, including mental health, learning disability, community physical health and all-age disability care, as well as the provision of three GP surgeries in Cheshire. We employ around 4,500 staff across 73 sites and have services across Wirral and Cheshire, as well as Trafford, Warrington, Bolton, Halton, Liverpool and Stockport. Our aim is to help improve the lives of everyone in our communities, adopting a compassionate, person-centred approach to everything we do. We are rated as Outstanding for Caring by the Care Quality Commission, with a Good rating overall. As a Disability Confident Employer, Rainbow Badge Scheme member, Veteran Aware organisation and proud holder of the Armed Forces Covenant Employer Recognition Scheme Gold Award, CWP is committed to making our recruitment practices as inclusive as possible and developing a culture that values differences, and welcome applications from people who have direct experience of accessing our services. All new starters will commence in post on one fixed start date each month, in line with our Prepare to Care (P2C) induction programme date. P2C offers up to 3 weeks training to support you to meet the induction / initial training requirements of the role. Whether you are just starting out in your career or looking to use your existing skills and experience in a new role, you'll find something to suit you at CWP. Details
