Job summary
An exciting and unique opportunity has arisen within our Pathway 2 Inpatient bedded rehabilitation , reablement & recovery services covering Magnolia unit ( St Michael's primary care centre site) and Capetown / Canterbury wards ( chase farm hospital site) for an experienced, confident and motivated clinician ( Registered nurse , Physiotherapist, Occupational Therapist or Speech & Language therapist) to join our rehabilitation MDT to ensure the service achieves timely screening and triaging of referrals into the P2 Bedded services.

The post -holder will provide expert advice to colleagues on rehabilitation pathways, eligibility criteria and patient flow through the services, liaising closely with Acute hospital teams, community services, patients and their families to co-ordinate smooth and safe transfers of care.

You will be part of vital patient flow initiatives and represent the P2 bedded services within flow and discharge meetings / forums to build collaborative relationships with system partners.

Main duties of the job
The post-holder (registered Nurse / AHP) will work in partnership with colleagues in the North Central London Intermediate care Hub (NCL ICEHUB) acute sector, social services, community health and primary care services via the to ensure that patient flow into and out of Pathway 2 bedded Inpatient wards - Magnolia unit, Capetown & Canterbury wards is managed within a robust quality agenda and supports patient flow in a timely and seamless manner. This role encompasses ensuring the timely screening of referrals for patients from the acute setting & community, to achieve safe transfers to North Middlesex University Hospital (NMUH) - P2 Inpatient bedded wards where the patient's needs can be best met.

The post - holder will be responsible for coordinating and managing day-to-day operations of the P2 bedded units' ward-based flow function and to act as a key point of contact to support complex admission and transfer queries.

Job responsibilities

• Provide highly skilled and autonomous oversight including taking responsibility for the assurance of timely referrals screening and assessment prior to admission to P2 bedded services, including liaison (external and internal) and reviews with regards to individual patient needs. This may entail ad-hoc assessment within an acute/ community setting for complex patients where needed.
• Take responsibility for management of patient flow into P2 in-patient beds including assurance of early identification of appropriate referrals, liaising with clinicians and discharge flow managers within referring locations to ensure patients are triaged effectively and directed to appropriate P2 bed bases where their care and rehabilitation needs will be best met.
• The post-holder will partake in a rota for daily/ weekly/ monthly feedback & reporting of required data sets related to patient flow performance indicators & represent P2 bedded services at sitrep and patient flow meetings.
• Act as the Team Leader for the P2 bedded wards flow function responsible for day-to-day referrals management; acting as a key point of contact for acute and community clinicians including Therapists, Nurses, Consultants, planned and unplanned team leads and other clinicians and local authority colleagues in the community in matters related to timely transfers of care.
• Work within and across professional and organisational boundaries, liaising with consultants, ward managers, nurse in charge, social services lead, other professionals and statutory and voluntary agencies to lead delivery of individualised and expert assessment and discharge guidance to a high standard and supporting communication with all stakeholders.
• Take designated responsibility for discrete areas of operational management/ clinical practice as identified by the service leads and Divisional leads.

Education and professional qualifications
Essential

• Registered Nurse with current NMC registration Or Registered AHP with current HCPC registration
• Evidence of continuing personal and professional development with up-to-date CPD portfolio
• Post registration clinical experience within acute/ inpatients/ rehabilitation services
• Post -registration qualification in advanced nursing/ AHP clinical practice, community rehabilitation, community nursing or equivalent relevant to the post
• Mentor or practice-based learning/clinical educator training

Desirable

• Evidence of continuing professional education and training at master's Level.
• Qualification in teaching or practice-based learning.
• Management or leadership qualification

Experience
Essential

• Evidence of significant recent post-registration experience in ward-based nursing or therapy, community practice, , discharge co-ordination or equivalent experience.
• Proven ability to prioritise and manage own workload with minimal supervision.
• Experience in referral triage, screening, Multidisciplinary liaison to support admissions and discharges within a service.
• Experience of effectively working in collaboration within a multi-disciplinary team, negotiating and liaising and across organisational boundaries
• Experience coordinating and delivering care for people with complex, long term conditions, preferably at the interface of acute and community services.
• Experience of leadership including coordinating and managing a team
• Experience of teaching others and mentoring students
• Experience in presenting and service promotion
