ACCOUNTABLE TO Head of Service Hospital at Home, ICS and older peoples ambulatory service and professionally HoS Physiotherapy Co-operation and Working Together (CAWT) is the cross border health and social care partnership, comprising the Health Service Executive in Ireland and the Southern and Western Health & Social Care Trusts, Strategic Planning and Performance Group and Public Health Agency in Northern Ireland. Formed in 1992, CAWT’s focus is on improving the health and social well-being of the resident population, through strategic cross border collaboration and the development and implementation of innovative and sustainable cross border health and social care services in partnership with the community and voluntary sectors. CAWT has recently been successful in securing significant funding to lead on the development and implementation of five strategic cross border PEACEPLUS projects in the areas of Addictions, Adult Mental Health, Children’s Mental Health, Geriatric Medicine and Obesity Management. The Early Frailty Intervention Project establish a new jointly developed Cross Border community-based Early Intervention Frailty infrastructure, to identify people living with/at risk of developing frailty earlier and jointly deliver early/preventative interventions within their local community to help slow down or reverse the process. Statutory partners, working collaboratively, will provide clinical support and episodes of health and social care to older people living with/at risk of developing frailty. The Early Frailty Intervention Project will provide a programme of care which could include the following areas; assessment, screening, education and community coordination sessions. We are now recruiting an Advanced Physiotherapy Practitioner to join a new team known as the Early Frailty Intervention Team. This team will comprise a Specialist Nurse, Physiotherapist, an Occupational Therapist and Community Connector. The service will focus on supporting people over the age of 55 living in the community with early signs of Frailty with the key aim of preventing progression of Frailty through early intervention, education, virtual monitoring and onward referral to appropriate services as necessary. Working closely with the specialist nurse, the occupational therapist and the community connector, the team will devise a bespoke programme of care for each individual based on their needs. The addition of the Early Frailty Intervention team will support the end to end pathway by identifying frailty at an early stage and working as part of a team to prevent progression, provide virtual monitoring where appropriate, education and appropriate interventions. The addition of a community connector will assist those who are isolated or have disengaged from local services to become connected again and support their wellbeing. JOB SUMMARY: The Early Frailty Intervention team’s primary focus is to support older people to live well in their community and contribute to the improvement of healthcare experience by focusing on early intervention and education to prevent the progression of Frailty. The Advanced Physiotherapy Practitioner will focus on assessing and promoting the health and function of older people living with early frailty, cognitive changes or falls risk by assisting them using virtual monitoring, education and bespoke programmes to support their wellbeing. The Advanced Physiotherapy Practitioner will link with other practitioner’s and services for onward referral of more complex cases which require a longer period of intervention and support due to complexity Clinical caseload will be for a short period of time extending from their well-being health needs through to the acute and chronic illnesses as they are manifested in later life. The Advanced Physiotherapy Practitioner will work as part of the Early Frailty Intervention Team to enhance the current model of care in the community ensuring older persons are supported to live well and age well with the appropriate supports and onward referrals. To facilitate patients and families understanding their care and to work in partnership with the community and hospital MDTs and wider communities to ensure a home first approach. To provide a seamless integrated service with multidimensional and multidisciplinary input for older persons as they transition through the continuum of care. To provide support in to community frailty services within the Trust to enhance focus and development of treatment of frailty
KEY DUTIES / RESPONSIBILITIES Clinical To be professionally and legally responsible and accountable for all aspects of your own work. To undertake specialist therapeutic assessment using advanced clinical reasoning, investigative and analytical skills to make an independent clinical diagnosis, in patients presenting with multi-pathology, recommending the best course of intervention and formulate individualised holistic management and treatment plans. To work as an expert practitioner and utilize specialist theoretical knowledge with cases of diverse presentations and those referred from other health care professionals for a second opinion/advice on further management To provide a high standard of care using safe effective, evidence-based interventions in accordance with current research and professional standards. To take a lead role in decisions re patient management, facilitating and planning appropriate onward referral, liaising with core community staff and specialist community teams. To provide highly specialist advice and education to colleagues, and a range of other Health and Social Care professionals, working across clinical areas. To maintain effective communication ensuring that information regarding assessment, prognosis, diagnosis etc is understood by the patient and their family for their long-term management. To ensure assessment of patient’s understanding of treatment proposals, goal setting. Gain valid informed consent and have the capacity to work within a legal framework with patients that lack capacity to consent to treatment To develop appropriate extended clinical practice skills. To represent physiotherapy, the service and /or individual patients at multidisciplinary meetings, locally and regionally to ensure the delivery of a co-coordinated MD service.
To develop Multi-disciplinary working to facilitate partnership care with Medics, Nursing, Occupational Therapy, Social Work and other agencies.
To undertake evidence-based audit and research projects making recommendations which may lead to the implementation of changes to practice or contribute to policy development, protocols/guidance.
To be an active member of the emergency out of hour’s rota as required.
To plan, implement, monitor and evaluate person centred interventions using individual and group techniques as appropriate in order to ensure effective outcomes. To advise and instruct patients, carers and family on safe use of equipment and implementation of rehabilitation programmes for their relative/client.
To work with inter-disciplinary colleagues to ensure the patient receives timely, appropriate and effective co-ordinated management and treatment, in line with the operational procedures for the Service, to impact in the long term on their lifestyle. Provide training and advice on changes and adaptations to their social and physical environment.
To give relevant advice and support to the patient/carer/family and a range of staff and partners on home management, liaising with the multidisciplinary/Integrated team. To manage clinical risk including own patient caseload and ensure safeguards are in place for other staff under your line management.
To be involved in the assessment and provision of a range of mobility aids and equipment, making appropriate recommendations for purchase or onward referral to other specialist services. PROFESSIONAL Comply with the Physiotherapy Code of Professional Conduct and national and local policies and procedures. Maintain own clinical professional development (CPD) by ensuring compliance with own PDP to meet set objectives and competencies. Comply with the codes of practice and registration guidance for Physiotherapy according to the regulatory body HCPC. To be an active contributor to the Team/Professional/ Trust in-service training programmes by delivering presentations and training sessions at staff meetings, tutorials and case discussions. Work within CSP clinical guidelines, monitoring own and evaluate others quality of practice through the use of evidence based practice, national guidance/frameworks and using validated outcome measures. Be actively involved in professional clinical groups such as Trust Journal club, CSP clinical interest groups, peer review groups. Completion of relevant MSc module e.g. Musculoskeletal, neurological, or willing to complete. Experience in delivering objectives which have led to a significant Improvement in Service. Experience in working with a diverse range of internal and external stakeholders in a role which has contributed to the successful implementation of a significant change initiative.
