Support Worker - Discharge to Assess
Location: South Ayrshire with travel around the area
Salary: £24,479 per annum based on 35 hours per week, pro-rata for roles under 35 hours (£13.45 per hour)
Hours: 18, 24 & 35 hours available
Contract: Permanent
Additional Requirement: Successful candidates will be required to register with the Scottish Social Security Council (SSSC) within 3 months of starting.

As a Support Worker , you’ll be at the heart of our regulated service, responding to referrals from health professionals and providing practical and emotional support to vulnerable adults. You’ll help prevent hospital re-admissions and promote wellbeing by enabling individuals to regain confidence and independence. What will a day in the life of a Support Worker involve?
- Visiting service users in their homes to assess needs and create tailored support plans.
- Assisting with daily living activities such as bathing, dressing, and meal preparation.
- Liaising with health, housing, and social care professionals to ensure consistent support.
- Carrying out welfare checks and helping individuals rebuild their independence.
To be a successful Support Worker, what will you need?
- Experience supporting people with physical, sensory, or mental health needs.
- Strong communication skills and the ability to work both independently and in a team.
- Knowledge of NHS and social care services, and how to access them.
- A flexible approach to working hours, including evenings and weekends.
