As a Qualified Practitioner within the Learning Disability South Team, you will play a key role in supporting adults with learning disabilities to live fulfilling, independent, and meaningful lives. You will be responsible for completing Care Act assessments, developing and reviewing support plans, and ensuring that individuals receive the right level of support at the right time. You will hold a current Nursing or Social Work registration. You will hold a varied caseload, including more complex cases, and will work with individuals on a longterm basis, enabling you to build strong, trusting relationships. As part of an experienced and enabling team, you will also have opportunities to supervise less experienced colleagues and contribute to learning and development of the team. - The Learning Disability focuses on working with adults who require assessments, Decision Support Tool, care planning, and ongoing interventions to meet their care and support needs. This often involves undertaking comprehensive assessments, developing person-centred support plans, and working collaboratively with families, carers, health professionals, and other partner agencies. The team covers Sleaford, Spalding, Grantham, Stamford and Bourne.
