Job summary
An exciting opportunity has arisen for a Band 6 Mental Health Practitioner to join our Core Team, based at Baghill House Health & Wellbeing Centre, Pontefract.

You will join a fully established multidisciplinary team, working in the community, promoting recovery and independence. We are a close knit, dedicated team, working in an often fast paced, high demand service, so passion and enthusiasm is essential.

We can offer you a comprehensive induction and regular 1:1 clinical support. This role offers the chance to work in an innovative trauma-informed way, undertaking meaningful clinical work as well as participating in the ongoing development of our trauma-informed pathway. We are keen to raise the standard of care for people who have experienced complex trauma, so welcome those who think creatively and dynamically about this.

One of the central features of our trauma pathway is our Structured Clinical Management (SCM) program; an evidence-based intensive intervention for people with complex emotional and relational needs (often those diagnosed with BPD/EUPD). It involves 1:1 and group working with a strong values-based framework around curiosity, validation and empowerment. We are keen to hear from people who have a good understanding of trauma and attachment, who are passionate about working from a strengths based, authentic clinical stance. A genuinely non-judgemental and compassionate perspective towards this client group is essential.

Main duties of the job
As a Mental Health Practitioner working in a Trauma Informed, treatment focused team you will be required to offer evidence-based interventions with a focus upon recovery and inclusion, such interventions will include SCM and DBT informed skills which will be delivered on both a group and individual basis. There will be an expectation to complete SCM training if not done so already.

Within your role your will also be expected to confidently and collaboratively undertake care planning and positive risk management whilst maintaining associated paperwork. You will be required to participate in the Team duty rota and will be offered regular Management and Clinical supervision to support in all aspects of your role.

Job responsibilities
JOB SUMMARY
Has continuing responsibility for the provision of skilled interventions to people with mental health problems in the community. Works within a multi-disciplinary team as a member of a Core CMHT.

Key Result Areas:
  • Management of own caseload in line with C.P.A. procedures and risk management protocols.
  • Provision of leadership to the Core Team in respect of psycho-social interventions to people and carers.
  • Supervision and support of other team members.
  • Participation in service development and clinical governance structures.
Professional:
  • 01. Assesses needs and identifies problems relevant to the care of people referred.
  • 02. Devises a plan of care in partnership with the person and carers.
  • 03. Implements the planned programme of care to ensure a high standard is achieved.
  • 04. Reviews the effectiveness of the care provided and where appropriate initiates any action.
  • 05. Advises and supports the person and carers to promote health and well-being and to prevent illness.
  • 06. Recognises situations that may be detrimental to the health and well-being of the person and initiates action that may be required.
  • 07. Establishes and maintains satisfactory communication with all agencies involved in the persons care in both hospital and community.
  • 08. Functions as a member of a multi-disciplinary team.
  • 09. Management of enquiries and direct referrals of people in accordance with the Core C.M.H.T. Operational Policy and participates in the team duty rota.
  • 10. Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.

Administrative:
  • 1. Maintains accurate patient care records in accordance with professional codes of conduct and trust procedures and ensures records are made available when required by the appropriate agency.
  • 2. Participates in the formulation of policies at unit level.
  • 3. Maintains accurate mileage records.
  • 4. Manages caseload with due economy of cost and time.
  • 5. Ensures safe carriage, storage and administration of drugs prescribed (registered nurses only).
  • 6. Responsible for the ordering of stock with due economy.
  • 7. Produces monthly statistics in line with trust directives.
  • 8. Keeps the Management Team and colleagues informed of developments relevant to the area of responsibility.
