Job summary
We are looking to appoint an experienced paediatric nurse into our Complex Discharge post. You must have extensive experience in caring for children with complex needs ideally in both an acute and community setting. You will have sound clinical skills and expertise in a wide breadth of nursing. You will need to provide leadership and act as a key worker to achieve excellence in care and discharge planning. The overriding focus will be on quality of life for children and young people with the recognition of the need to be inclusive when making plans. Seeking always to reach the best care solutions within agreed time frames. Cambridge has a Paediatric Intensive Care that accommodates children across the whole of the East of England so you will be dealing with hospitals and services across the region. You will be working alongside many specialist multi disciplinary teams and act as a point of contact to facilitate children and young people either returning to their homes, specialist rehabilitation centres or their local hospital. Therefore we are looking for an applicant with outstanding communication and organisational skills. The post holder will undertake clinical work and support the inpatient areas to facilitate smooth patient journey's and to avoid any delays in discharge. The post holder will work across professional systems, organizational and geographic boundaries to facilitate the delivery of the key objectives.
