Paediatric Complex Discharge Nurse

Job summary
We are looking to appoint an experienced paediatric nurse into our Complex Discharge post. You must have extensive experience in caring for children with complex needs ideally in both an acute and community setting. You will have sound clinical skills and expertise in a wide breadth of nursing. You will need to provide leadership and act as a key worker to achieve excellence in care and discharge planning. The overriding focus will be on quality of life for children and young people with the recognition of the need to be inclusive when making plans. Seeking always to reach the best care solutions within agreed time frames. Cambridge has a Paediatric Intensive Care that accommodates children across the whole of the East of England so you will be dealing with hospitals and services across the region.

You will be working alongside many specialist multi disciplinary teams and act as a point of contact to facilitate children and young people either returning to their homes, specialist rehabilitation centres or their local hospital. Therefore we are looking for an applicant with outstanding communication and organisational skills. The post holder will undertake clinical work and support the inpatient areas to facilitate smooth patient journey's and to avoid any delays in discharge. Providing specialist advice and support to staff, including the provision of teaching and training.

This is an exciting time to join Cambridge University Hospital as our new purpose built Childrens Hospital that will accommodate both physical and mental health is due to open in the next few years.

Main duties of the job
The post holder will provide leadership and act as a key worker to achieve excellence in care and discharge planning. They will deliver the key NSF standards for children with complex needs. The overriding focus will be on quality of life and care for children and young people with a full recognition of the need to be inclusive when making good plans. The post holder will focus on seeking the best care solutions within the agreed time frame, challenging, where appropriate, views on patient/carer risks and resilience. The post holder will work across professional systems, organizational and geographic boundaries to facilitate the delivery of the key objectives.

Responsible for providing specialist advice and support to, and liaising with, other staff. This includes the provision of specialist education and training for other staff and students

Responsible for implementing a clearly defined integrated approach to patient care encompassing new treatments and evidence based care

Required to undertake research and lead clinical audits in own specialist area
