We have an exciting opportunity for a Band 6 Community Psychiatric Nurse (CPN) to join the Primary Care Team in Weston-super-Mare . This full-time locum role offers a competitive pay rate of up to £26 per hour over an initial 3-month contract . What you will do:

Carry out comprehensive mental health assessments, identifying the nature and severity of mental health conditions and differentiating between physical and psychological symptoms.
Develop, implement, and review person-centred care plans that promote recovery, independence, and positive mental health outcomes.
Work collaboratively with GPs, primary care professionals, social care teams, and other partner agencies to provide coordinated care.
Deliver specialist mental health interventions while working autonomously and exercising sound clinical judgement.
Undertake carers' assessments and provide support and education to families and carers where appropriate.
Mentor, supervise, and assess students and junior staff, supporting their professional development.
Apply current mental health legislation and relevant legal frameworks to ensure safe, effective, and compliant practice.
Maintain accurate clinical documentation and utilise electronic systems to support efficient service delivery.
Communicate effectively with patients, carers, colleagues, and external agencies, demonstrating excellent verbal and written communication skills.
