Job summary
Are you a dynamic and dedicated Physiotherapist seeking a rewarding opportunity to make a significant impact on the lives of individuals within Hywel Dda University Health Board? We currently have a unique opportunity for a highly motivated, driven and passionate Physiotherapist to join our dynamic Cardiac Rehabilitation team in Carmarthenshire. The post-holder will act as an autonomous practitioner, providing specialist physiotherapy assessment and treatment, manage their own patient caseload with the support and mentoring of senior physiotherapists and nurses. The post holder will work closely within an MDT to deliver group and individual based exercise, education programmes, assessments and telephone reviews.

Innovative practice is supported, and service development is actively encouraged. Training and support will be provided and readily available.

This post is a Fixed Term/ Secondment until 31.08.2027 due to - meet the needs of the service

Main duties of the job
The successful candidate will provide a comprehensive assessment of patients including those with diverse or complex presentations/multi pathologies and use advanced clinical reasoning skills to devise an evidence-based treatment plan.

The post holder will communicate with and work alongside physiotherapists, nurses, occupational therapists, secretaries, wider members of the MDT and other community services.

The post holder will lead both group and individual exercise sessions, provide personalised home exercise programmes, and support patients to achieve their rehabilitation goals. They will utilise validated outcome measures and comprehensive risk assessments to monitor progress, optimise patient outcomes, and ensure the safe and effective delivery of cardiac rehabilitation interventions.

The post holder will be expected to complete all appropriate electronic information and discharge summaries as well as database entries where relevant for audit and service evaluation. Basic IT skills are essential.

The successful candidate will have the opportunity to undertake the British Association of Prevention and Rehabilitation (BACPR) Physical Activity and Exercise in the management of cardiovascular disease part 1 and 2 course, to support the development of specialist knowledge and skills.

Qualifications and Knowledge
Essential

• Degree in Physiotherapy
• Current HCPC registration
• Relevant postgraduate qualification or evidence of study at postgraduate level
• Demonstrable specialist knowledge relevant to the clinical area
• Evidence of relevant post-registration continuing professional development.

Desirable

• Practice educator or clinical education qualification.
• Membership of the Chartered Society of Physiotherapy (CSP)
• Active involvement in specialist professional groups

Experience
Essential

• Post-registration experience within a relevant clinical specialty
• Experience of managing a diverse/complex caseload using advanced clinical reasoning skills
• Experience of multidisciplinary team working
• Experience of risk assessment and clinical decision-making
• Experience of supervising, supporting or developing staff, students or support workers
• Experience of contributing to clinical governance, audit or service improvement activities

Desirable

• Experience of service development or redesign
• Experience of student education and placement supervision
• Experience of NHS working.
