Job Summary
**Please note this is a part time job for 18.75 hours** (07:15-18:00 day, weekends and bank holidays). The ASK team is highly valued across the Trust and plays an important role in improving the treatment, escalation, and care of AKI and Sepsis patients.The post holder will works clinically as part of theAcute Sepsis and Kidney Injury (ASK) Team, providing specialist assessment, review, and management of patients with Acute Kidney Injury (AKI) across the Trust. The role includes proactive daily review of AKI patients using the electronic patient record and AKI alert system, ensuring early recognition, timely escalation, and delivery of evidence-based care in line with NICE guidance and Trust policies. The post holder supports optimisation of treatment, monitoring of progress, and coordination with multidisciplinary teams to prevent deterioration and improve outcomes. The post holder acts as a clinical expert and resource for ward and front-door teams, providing advice, education, and support to embed best practice, reduce variation in care, and promote safe discharge. The role contributes to audit, quality improvement, and ongoing development of AKI and sepsis pathways, supporting a consistent seven-day service and high-quality patient care. The ASK Team provides comprehensive Sepsis training across the Trust and community settings and is leading a significant quality improvement initiative to implement the new Sepsis Tool, ensuring alignment with the latest NICE guidance. Main duties of the job
- Provide specialist clinical assessment and management for patients with Acute Kidney Injury, ensuring timely identification, treatment using the SHOUT bundle.
- Work proactively across front-door areas and inpatient wards to identify deteriorating patients, initiate appropriate interventions, and ensure prompt senior reviews.
- Support staff in the early recognition of sepsis using the Sepsis Six tool, guiding clinical teams to deliver timely treatment in line with NICE guidance.
- Deliver high-quality education and training to multidisciplinary teams on AKI prevention, management, sepsis recognition, and treatment.
- Lead and contribute to quality improvement projects relating to AKI and sepsis care.
- Work collaboratively with ward teams, the Emergency Department, and specialty services to enhance early intervention and integrated care.
- Provide expert advice on fluid management, medication, and treatment plans for patients with or at risk of AKI.
- Support the delivery of a seven-day service, ensuring equitable specialist cover, continuity of care, and timely patient review.
- Guide safe discharge planning and follow-up for complex AKI patients, including telephone follow-ups to support recovery and prevent readmission.
- Act as a clinical role model, demonstrating advanced nursing practice, patient advocacy, and high standards of care.
- Participate in policy development, pathway refinement, and implementation of NICE-aligned best practice for AKI and sepsis management.

- Relevant clinical course to enable history taking, and clinical examination, diagnostic reasoning, and differential diagnosis

- Demonstrable post qualification experience within an acute environment
- High standard of nursing care and clinical knowledge
- Evidence of continued professional development
- Computer Literacy
- Experience of managing acutely/critically ill patients
- Experience of delivering the Sepsis 6 and/or SHOUT bundles

- Demonstrable experience of delivering teaching/training
