Job Title: Band 6 Occupational Therapist - Rapid Response Team

* Provide rapid response interventions to support stabilisation during acute decline.
* Assess functional ability and environmental factors within patients' homes.
* Implement strategies and equipment to safely maintain independence.
* Collaborate with multidisciplinary team (MDT) colleagues for crisis intervention.
* Support safe discharge pathways to reduce avoidable delays in care transfer.
* Coordinate with health, social care, and voluntary services for community support.
* Develop person-centred plans to maximise function and mitigate admission risks.
* Monitor outcomes and adjust interventions to support community recovery.
* Maintain detailed clinical records and contribute to performance targets. Requirements * Degree in Occupational Therapy.
* HCPC Registration.
* Prior NHS Occupational Therapy experience.
* Post-qualification Occupational Therapy experience.
* Community and Rapid Response experience preferred.
* Skills in falls assessment and risk management.
* Experience in admission avoidance and discharge support pathways.
* Strong clinical reasoning and decision-making abilities.
* Ability to work autonomously in community settings.
