Our vibrant unit is dedicated to getting patients home safely and swiftly. With a cozy discharge lounge for patients awaiting their journey home, we focus on preventing de-conditioning and minimizing hospital stays.

Your Role:
As a key member of our team, you'll help plan our patient discharges, ensuring each patients needs are met and supported with a passion to embrace the patients day. Your enthusiasm will shine through as you engage patients in fun, uplifting activities like arts & crafts, card games, and even strolls outside, all designed to promote independence and well-being.

Main duties of the job

• Assess, plan, implement, and evaluate care programs under the guidance of senior staff.
• Supervise and support unqualified staff while working independently.
