An exciting opportunity has become available to join the Short-Term Services Team. We are currently recruiting for two full-time positions and one part-time position. The part-time role is for 0.8 FTE (29.5 hours per week).
These vacancies are open to both progressed and non-progressed Social Workers.
About you
The Short-Term Services teams are aligned with Leighton and Macclesfield Hospitals and work in close partnership with Mid Cheshire Hospitals NHS Foundation Trust and East Cheshire NHS Trust. The service undertakes proportionate assessments to support timely hospital discharge, working alongside hospital therapists to assess short-term care and support needs, and developing person-centred recovery and support plans with individuals and their families or carers, including Reablement and equipment provision. The team also manages patient flow through discharge pathways and supports individuals where safeguarding risks are identified, working closely with health colleagues. As a Social Worker, you will undertake adult needs assessments in line with the Care Act (2014), manage and respond to safeguarding concerns, apply the Mental Capacity Act (2005) to assess capacity and make best interest decisions, deliver person-centred and evidence-based practice, manage a diverse and complex caseload, work collaboratively with multi-disciplinary teams and wider Adult Social Care Teams.
The successful candidate will have:
- Strong understanding of adult social care frameworks;
- Excellent communication and interpersonal skills;
- Ability to work autonomously within policies and professional standards;
- Effective time management and organisational skills for caseload management;
- A proactive and self-motivated approach.
