£39.96k - £48.12k per annumFull-time
Job summary
Due to an exciting service expansion we are looking to appoint a highly skilled, enthusiastic and forward-thinking Senior Community Children's Nurse to our Community Children's Nursing (CCN) team. Our established, passionate and dedicated CCN team work collaboratively with children, young people, families and the wider multi-disciplinary team to ensure care is provided as close to home as possible. We provide high quality and holistic, family centred care to children and young people (CYP) with a nursing need across the Southampton and west Hampshire area. As a Senior CCN you will have the opportunity to shape and lead our responsive service as it continually adapts to the ever changing needs of the families we care for. You will have a key role in leading care planning, training and multi-disciplinary liaison for CYP with a range of needs within their home, school and wider community settings. If our Senior Community Children's Nurse role appeals, you have the qualities we are looking for, share our values and feel excited by the prospect of how we can complement your professional career then we'd love to hear from you. We welcome you to contact us prior to interview to discuss this post further and to arrange a suitable time to come along for an informal visit. For more information please contact View email address on meeveem.com or View email address on meeveem.com Main duties of the job
Manage a caseload. Provide specialised, high quality skilled nursing care, considering the physical, psychological, spiritual and social needs of the child with nursing care needs. This will include those with complex health care needs, palliative care needs, life-limiting illnesses and children dependant on technology (and those requiring short term support). This requires developing, implementing and evaluating individualised programmes of care. Teach families/carers to carry out specific complex nursing care and procedures which enables them to feel confident and competent in caring for the child. Participate in the administration of medicines including intravenous therapy and cytotoxic drugs, in partnership with pharmacy colleagues. Be aware of current drugs/therapies in the treatment of pain and other symptoms. This requires specialist knowledge of children's palliative care issues and symptom management. Maintain regular liaison with local paediatric unit, paediatric emergency department, primary health care team and other agencies to ensure good communication and continuity of care for the child. Working collaboratively with all professionals to enable children/young people to be cared for at home whenever possible. Participate fully in the on-call rota with other Children's Community Nurses and to take responsibility for providing end-of-life care out of hours as required. About us
Hampshire and Isle of Wight Healthcare NHS Foundation Trust provides joined-up mental and physical healthcare for around two million people across our communities. With over 13,000 staff working in the community and local hospitals, we deliver care at every stage of life, helping people live their best and healthiest lives. Our mental health services include community-based support and early intervention in psychosis (EIP) for both adults and young people, alongside a network of specialist inpatient wards covering forensic, learning disability, eating disorder and older person's care. We deliver extensive physical health services too, from urgent community response teams helping frail and older patients remain safely at home, to hospitals at home teams providing acute-level care in familiar surroundings. Our neurological services offer rehabilitation and treatment for conditions including Multiple Sclerosis, Parkinson's Disease, Motor Neurone Disease, Head Injury, Cerebral Palsy and Stroke. Across Hampshire, our community hospitals provide inpatient rehabilitation as a step down from acute care, and our dedicated teams also staff Treetops Sexual Assault Referral Centre in Portsmouth, offering expert, compassionate support. Everything we do is underpinned by our CARE values of compassion, accountability, respect and excellence Details
