Specialty Doctor in Elderly Care Medicine

Job summary
The Department of Medicine for the Elderly has evolved and developed with many innovations in Care of the Elderly services over the last few years. There are currently 5 Consultant Geriatricians, including a Stroke Consultant, an Orthogeriatrician, and 3 general Geriatricians with special interests in movement disorders and falls.

At present we have one 30 bedded Care of the Elderly ward in Ysbyty Gwynedd in addition to a fully functioning 13 bedded Acute Stroke unit. In addition to the above beds in Ysbyty Gwynedd, we are responsible for the inpatient beds in Eryri and Ysbyty Penrhos Stanley Community Hospitals, which includes the new Early Supported Discharge (ESD) team and a new stroke rehabilitation unit at Eryri Hospital. We also provide Geriatrician input on request for our GP colleagues in the other Community Hospitals.

The Community Hospitals provide a valuable opportunity to provide CGA, rehabilitation and intermediate care, as well as enabling crisis intervention to frail older people who are otherwise at risk of an unplanned emergency admission to the acute hospital.

Main duties of the job
The appointee will be expected to contribute with consultant colleagues and peers to the provision of the service within the department by the following duties:
  • Specialty ward round and outpatient clinics (movement disorder)
  • To be actively involved in the management and decision making for patients
  • To conduct ward rounds to identify and assess problems and assist in joint plans for management of care.
  • To contribute with the multi-disciplinary team.
  • Ensure that an appropriate model of care is adopted which recognises the involvement of the multi-disciplinary team, the patient and family.
  • Support the delivery of a high-quality service provided by well-trained staff within clear departmental policies and procedures.
  • Develop and maintain good communication with clinical colleagues across the Health Board, regarding the treatment of their patients, and the appropriate use of the department's resources.
  • Required to participate in the Health Board Risk Management process and in Clinical Audit and other clinical governance activities within the department.
  • In liaison with senior medical colleagues have responsibility for the proper functioning of the department in line with the operational policy and strategic plan of the Health Board
  • Participate in a multi-disciplinary structured programme of training and education

Qualifications
Essential
  • GMC registration
  • Licence to practise in the UK
Desirable
  • MRCP, MRCGP or equivalent
Clinical Skills and Experience
Essential
  • Experience and training in COTE and movement disorder service
  • Previous audit or research project
  • Familiarity with NHS in the UK
  • At least 4 years postgraduate experience with a minimum 2 years in the specialty
Leadership, Management and Administrative Experience
Essential
  • Ability to work effectively with colleagues in an MDT setting
Personal qualities
Essential
  • Ability to work in under-pressure and deal with stressful situations.
  • Good communicator
