What you will do:

Conduct timely and proportionate assessments and reviews in accordance with the Care Act for residents living in Extra Care Housing.
Guide and support residents through the transition process, ensuring their care and support plans are updated to reflect the latest accommodation changes.
Collaborate effectively with housing providers, incoming and outgoing care providers, commissioners, and health professionals to maintain continuity of care.
Identify potential risks arising from changes in care provision and accommodations, implementing appropriate risk management and safeguarding measures.
Perform Mental Capacity Assessments and participate in Best Interest decision-making processes when necessary.
Keep residents, their families, and advocates informed and supported throughout the transition process.
Maintain accurate and timely case records, adhering to statutory and organisational standards.

Essential Requirements:

Qualified Social Worker with registration from Social Work England.
Recent experience in adult social care, particularly in settings dealing with older people or housing-based support services.
Strong understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding responsibilities.
Confidence in handling a complex caseload independently, especially in transitional environments.

Desirable:

Experience in managing service transitions or provider changes.
Proven ability to work in partnership with housing providers.
Familiarity with commissioning processes and care provider handovers.
