We are looking for an experienced and motivated Advanced Nurse Practitioner to join our dynamic and working closely with the leadership and practice teams. Youll be involved in the smooth day to day running of the practice and pro-actively engaged in assisting both practices to achieve its vision and objectives.

Please note that this role is offered at 16 hours per week.

The full-time equivalent (FTE) salary range is £51,954 - £63,706 per annum.

The part-time equivalent salary range for 16 hours per week is £22,670 - £27,786 per annum.

Main duties of the job
Advanced Nurse Practitioners (ANPs) in primary care provide expert assessment, diagnosis, treatment and ongoing management of patients, working within their professional scope of practice.

• An ability to make professional autonomous decisions for which they are accountable.
• An ability to assess, diagnose and treat patients with undifferentiated and undiagnosed problems.
• Screen patients for disease risk factors and early signs of illness.
• Make differential diagnoses using advanced decision-making and problem-solving skills.
• Develop, with the patient, an ongoing care plan with emphasis on health education and preventative measures.
• Order and follow up on necessary investigations, providing treatment and care both individually and as a team.
• Work collaboratively with healthcare professionals across primary and secondary care, referring to other agencies where appropriate.
• An ability to provide leadership, education and mentorship.
• An ability to provide clear and accurate documentation.
• An appreciation of the importance of confidentiality.
• The ability to assess safeguarding concerns and proactively refer to relevant organisations and safeguarding teams.
• An acknowledgement of the importance of equality, diversity and the rights of patients, carers and colleagues, demonstrating a non-judgemental and respectful approach to each individual's circumstances, feelings, beliefs, priorities and rights.

The New Queen Street & Stanground Team consists of 9 GP Partners, 2 ANPs, 6 nurses, 4 HCAs, plus multiple teams (prescriptions, admin, reception) as well as a supporting management team.

We help over 19k patients in the surrounding areas and are part of South Peterborough PCN.
