You will deliver an advanced clinical pharmacy service across specialist wards, playing a central role in the Intestinal Failure Service -- supporting PN ward rounds, optimising patient-specific nutrition regimens, and providing expert pharmaceutical guidance to prescribers, nursing staff, and the wider MDT. This isn't a back-office role; you will be a visible, trusted clinical expert.

Beyond nutrition, you'll rotate across a range of specialties, broadening your clinical expertise while achieving meaningful, measurable performance objectives. You'll contribute to audit and research, shape drug treatment guidelines, train and mentor junior colleagues, and help drive service innovation across the Trust and into the community.

Main duties of the job

• Following appropriate training, actively participate in and lead PN ward rounds twice weekly to review patients' needs and place orders for required PN.
• After appropriate training and accreditation, participate as a non-medical member of the Intestinal Failure Service (IFS) team to order safe and effective PN for adult patients.
• Assisting with checking clinical trials and specials.To liaise closely with pharmacy staff to answer any PN enquiries relating to individual patients.
• To contribute to the development and maintenance of clinical care plans, policies and procedures for PN in liaison with the nutrition team.
• To provide advice on all pharmaceutical aspects of PN care of patients to the IFS team, prescribers and nursing staff.
• To be an active member of the IFS team; to develop and agree objectives compatible with the nutrition team and pharmacy department.To liaise closely with pharmaceutical preparative services (PPS) in service development issues concerning PN.
• To assist in the development and to undertake projects to review and optimise PN use across the Trust and in the community.
• To participate in audit, clinical and practice research relating to PN as appropriate within the Trust and community.
• To develop and maintain appropriate information gathering mechanisms and to use this information for quality of care reporting, risk management and business planning. To support budget holders with queries and reports regarding the Trust spend on PN.

Qualifications
Essential

• Masters level degree in pharmacy.
• Post-graduate preregistration year.
• Registered with General Pharmaceutical Council.
• Completed Certificate (Year 1) of Diploma in Clinical Pharmacy/General Pharmacy Practice and studying for completion of year 2 or completed equivalent qualification as assessed by the interview panel. For candidates who have not completed year 2 of the diploma an academic reference will be sought in addition to the two standard references.

Desirable

• Completed year 1 and year 2 of Diploma in Clinical Pharmacy/General Pharmacy Practice)*.
• MSc in Clinical Pharmacy (or equivalent)*.
• Qualified and registered as an Independent prescriber.
• Risk management training.
• Professional and final accuracy checking.
• Member of the faculty of the Royal Pharmaceutical Society (or working towards)

Experience
Essential

• Moderate post-registration hospital experience including prescription screening in inpatients/outpatients, medicines reconciliation, medicines information, experience of preparative services either TPN/chemotherapy.
• Experienced clinical pharmacist who has covered a range of specialities. Medicine including elderly care, acute medicine, and including specialist medicine and Surgery including general surgery and other surgical specialities
• Documentation in medical notes.
• Writing reports
• Writing or assisting in writing drug policies and guidelines/PGD development.
• Evidence of having conducted a clinical audit.

Desirable

• Recent post-registration hospital experience, ideally including previous experience in a teaching hospital and experience of more than one hospital.
• Experienced clinical pharmacist (at band 7) including one or more of the following neurosciences, paediatrics, critical care, renal, transplant, oncology, haematology, nutrition
• Evidence of having written pharmaceutical and/or educational publications.
