Neighbourhood Health Programme Manager

Are you a skilled leader who aspires to a rewarding role transforming care pathways? We are looking for two experienced programme managers with an interest in system working, to lead work with partners across the Leeds Health & Care system to deliver our local ambition to develop a Neighbourhood Health approach

You would lead delivery of a complex transformation programme in one of the following areas:

• Active Recovery & Stroke Pathway: responsible for the expansion and strengthening of the existing Active Recovery model and implementation of an early supported discharge pathway for survivors of stroke.
• Neighbourhood Diabetes Support Team Model: responsible for development and testing of a new model of integrated, community-based specialist diabetes care.

We welcome applications on a secondment basis from anyone applying from within the Leeds Health & Care Partnership. Roles are offered fixed-term for 12 months and may be subject to extension in 2027/28.

You will work on behalf of the Leeds Health & Care system, hosted by Leeds Community Healthcare Business, Change & Improvement Service - a network of skilled and experienced project managers, change agents and business managers. You will work alongside a project manager and project support officer as part of a smaller team.

You will use programme management and quality improvement expertise to lead and monitor your programme area and drive transformational change. You will ensure the programme meets agreed milestones and intended outputs and outcomes, reporting progress through effective governance mechanisms. As part of a complex scheme of work, you will work with wider system workstreams under the Neighbourhood Health Programme to manage key interdependencies to deliver success.

A key aspect of the role is engagement and relationship management of multiple stakeholders across the health, social care and third sector including people (patient) representatives. This will involve application of your advanced facilitation skills to co-produce solutions and design systems and processes. You will also use critical skills in influence and negotiation.

You will set up an evaluation framework to quantify, monitor and track benefits, using this learning to update the scheme business case and demonstrate the ongoing value of investment.

Knowledge and understanding of local intermediate care rehabilitation pathways (including the stroke pathway) and/or the diabetes care pathway is what will set you apart as the best candidate to lead this critical work for the local area.
