Contract role, Community Adult Social Care Team Pertemps are recruiting for experienced Adult’s Social Worker in West London
This to work in a Community Adult Social Care Team in the Uxbridge area.
Key Responsibilities
- Manage a caseload within a community adult social work setting.
- Undertake Care Act Assessments, reviews and support planning.
- Complete Mental Capacity Assessments and Best Interest decision-making.
- Lead and contribute to Adult Safeguarding enquiries, including Section 42 investigations.
- Undertake Section 117 reviews and contribute to multi-agency planning.
- Work collaboratively with health professionals, providers, families and partner agencies.
- Promote independence, strengths-based practice and positive outcomes for residents.
