Unplanned Care - Community Staff Nurse (Band 5) Are you a passionate and caring fully qualified nurse looking to work in a dynamic community nursing team? If so, then we would like to meet you. A vacancy has arisen within the Unplanned Care Team, which has an innovative approach to nursing care. It involves quick responses to patients in their home to assess and manage the presenting issue, prevent unnecessary hospital admissions, and help to facilitate early hospital discharges. We are looking for a highly motivated applicant who is interested in meeting the challenge of working in the community with patients in their home setting. As part of the Unplanned care team, you will be working with a variety of disciplines within the multidisciplinary team to achieve the best outcomes for patients. You'll be working on a rotational basis as part of a seven-day service covering 24-hours-a-day with the support of agile technology to help you deliver timely, high-quality care.
